Membership Monthly Fees:
$80/month for individuals 25 and older
$150/month for married couples
$175/month for families of 3 or more (children 18 and under)
$30/month for ages 0-25
$125 per home visit. -Membership is also required.
Initial Membership Signup Fees:
$75 Each patient
$150 Married Couple
$150 Family Membership 3+ (children 18 and under)
NOTE: The one-time initial membership signup fee is waived if the first year is paid in full at the onset of membership.
